1. Acceptance of Terms

By accessing this website, booking an appointment, or receiving services from Complete Vitality Path ("Complete Vitality Path," "we," "us"), you agree to these Terms & Conditions. If you do not agree, please discontinue use of the site and services. These terms apply to all visitors, patients, and members.

2. Services Overview

Complete Vitality Path provides primary medical care, dental care, weight-loss programs, fitness and wellness services, hair and skin care, holistic wellness programs, and insurance guidance through licensed advisors. Insurance products are offered through separately licensed partners. All clinical services are delivered by licensed, board-certified professionals.

3. Appointments & Cancellation

We ask for at least 24 hours' notice for cancellations or reschedules. Repeated missed appointments without notice may require a prepayment for future bookings. Telehealth visit links expire 15 minutes after the scheduled start time.

4. Insurance, Billing & Payment

We verify coverage before treatment and provide written cost estimates on request. You are responsible for copays, deductibles, and non-covered services. Payment plans and HSA/FSA payments are accepted. Complete Vitality Path never bills surprise out-of-network charges for in-facility care.

5. Telehealth Consent

By using video visits, you consent to electronic delivery of care, understand that some conditions require in-person examination, and agree that telehealth encounters are documented in your medical record like any other visit.

6. Wellness Program Disclaimer

Wellness Studio programs — including nutrition plans, herbal guidance, and mindfulness training — support general wellbeing and do not replace medical diagnosis or treatment. Always follow the advice of your primary provider for acute or serious conditions.

7. Acceptable Use

You agree not to misuse this website, attempt unauthorized access to patient portals, submit fraudulent insurance information, or use our content commercially without written permission.

8. Limitation of Liability

To the fullest extent permitted by law, Complete Vitality Path's liability arising from use of this website is limited to direct damages not exceeding fees paid for the related service. Nothing in these terms limits liability that cannot be limited by law, including claims arising from clinical negligence.

9. Changes to These Terms

We may update these terms periodically. Material changes will be posted on this page with a revised "last updated" date. Continued use after changes constitutes acceptance.

10. Governing Law

These terms are governed by the laws of the state in which the applicable services are provided, without regard to conflict-of-law principles. Any disputes will be resolved in the courts of that state.
